Thank you for entrusting Jersey Premier Pain, LLC (the “Practice”) with your medical care. Effective on August 23, 2024, the Practice will cease performing patient care services. From and after the Effective Date, your provider will continue to provide services at 550 Newark Ave, Ste 201, Jersey City, NJ 07306 under the employ of ALLCARE HEALTH & PAIN, LLC (the “New Practice”) and remains ready, able, and willing to continue to serve as your treating provider. To the extent that you are scheduled to treat with your provider after the Effetive date, the New Practice will honor that appointment and you may report to the Practice Location for such appointment.
If you would like to continue to treat with your provider under the New Practice, you need not do anything. If you would like to obtain or transfer a copy of your medical records to another health care provider(s), or if you have any questions, please contact us at [201-386-9800].
